fix(platform-integrations): scope Claude-only doctor skill out of codex/bob plugins
doctor's @import-canary diagnostic greps ~/.claude transcripts and is Claude-specific, so it's meaningless on codex/bob (Codex uses an ~/.codex/AGENTS.md pointer); exclude it from those plugins (and bob's auto-generated command), addressing CodeRabbit's Critical on PR #266.
