Add maintenance-mode announcement to documentation

Overview:

Make it clear that this package is in maintenance mode.

Addresses:

Lack of recent development.

Type of Fix:

• Bug fix (non-breaking change which fixes an issue)
• New feature (non-breaking change which adds functionality)
• Breaking change (fix or feature that would cause existing
  functionality to not work as expected)
• Documentation Change

Solution:

Manage user's expectations with maintenance mode announcement.

Changes:

Update README.md, CONTRIBUTING.md, and PR template.
