Remove duplicate captions, add Module 16 illustrations, archive HTML agents
- Remove 297 duplicate code captions across 124 files (auto-fix script regression)
- Fix root cause in check_missing_code_captions.py (code-output lookahead)
- Add fix_duplicate_captions.py detection script
- Generate and embed 6 new illustrations for Module 16 (distillation/merging)
- Refine 10 remaining TODO placeholder captions to proper descriptions
- Archive 42 HTML agent files to agents/archive/, use .md as single source
- Update CLAUDE.md to reference agents/book-skills/agents/*.md
- Sync Value Gate section to code-pedagogy agent .md
- Add book-wide run guidance to code-caption agent .md
- Update MetaAgent to allow direct skill file edits
