[Feature]: Better observability for refinements

Problem Statement

Currently when a task is done by the agent but we want to make refinements, the refinement prompt and any logs of the agent working on said refinement seem to be lost and only the original task is shown. It would be very nice to have more observability on if a task has been refined and what work from the agent has been done to it
