feat: improve application insights logging and telemetry handling

Purpose

• This pull request introduces robust telemetry and logging improvements across the application, focusing on enhanced Application Insights integration, propagation of conversation and user context in telemetry, and targeted suppression of noisy Azure SDK logs. Additionally, it adds custom event tracking for key conversation and chat history actions, and ensures these features are conditionally enabled via infrastructure configuration.
