refactor: remove Part 7 go-live section and appendices H/L

- Delete 07-go-live directory (delivery confirmations, cluster handover, checklists)
- Remove Appendix H (B2B sync templates) and Appendix L (handover artifacts)
- Update all cross-references in docs index, implementation index, Part 5, Part 6
- Fix misdirected links from operational-foundations pointing to 07-go-live
