Implement entry level augmentation according to the spec described in #63
Why it needs to be done
After discussion with APHL we're moving to entry-level augmentation instead of a header-level summary.
Acceptance Criteria
- For changes within
clinicalStatements: Changes flagged by a config should result in an author entry for that clinical statement with the functionCode indicating the type of change
- For changes outside of
clinicalStatements: forgo augmentation for that change.
