docs(v4-plan): all 11+ ROIs now have committed code + tests
Final status snapshot: 7 ROIs landed as full minimal-edit upstream ports with numerical parity (1, 2, 3.A, 3.5.A, 4, 5, 6). 6 ROIs (3.B-F, 3.5.B-D, 3.7, 7, 8, 9) landed as scaffolds with stable API + dispatch + tests, falling back to Path A reference until their kernels land. 166/166 v4 tests + 147/147 regression green at HEAD c00c62b. cppmega_mlx/ remains untouched; cppmega_v4/ is the side-by-side plugin.
