chore(tracker): decompose axon-11470d3b into 6 per-surface child beads

Verification found NEEDS_BUILD across all 15 cells (5 surfaces × 3
versions). Decomposed into:

- axon-d73680e3: extend GraphQL auditLog with intentLineage versions
  (closes REST-only fallback gap on audit-lineage)
- axon-351b9ec5: policy-authoring version indicators
- axon-080efad0: policy-enforcement version indicators
- axon-d2a2a33c: mutation-intents version indicators
- axon-c812ae66: approval-inbox version indicators
- axon-b60bc2e6: audit-lineage version indicators (deps on d73680e3)

axon-11470d3b deps += all 6, becomes a tracking bead.
