Make guideline items identifiable

Sorry, that wasn't clear. I meant per item.
This could also help to integrate the items into the given or an even more sophisticated hierarchy.

The document can be taken care by the usual github versioning

How about a simply X.Y.Z numbering? Do you think that would do?
