• W9 repair commit cc4eedfb3 (this branch head):
  • Converted to canonical 4-column header
  • Added row numbers 1-28
  • Populated evidence cells per row
  • Removed redundant NO_PAYOUT row (subsumed by NO_PAYOUT_READY)
  • Substantive content (model, scenarios, M2J recommendation, HoloIndex assessment) unchanged
