feat(wiki): add two-pass survey, coverage cross-check, and ordering rules

- Add deep code scan (Pass 2) for feature discovery via grep
- Add survey context document output for sub-agent sharing
- Add coverage cross-check reconciling discovered topics with directory enumeration
- Add page ordering enforcement rules for sidebar consistency
- Add page title rules (concise noun phrases, no path prefixes)
- Add depth guidance with concrete examples for sub-page splitting
- Cap nesting at 2 levels from lens root
