Add CI/CD workflows

• Add GitHub Actions CI workflow (lint, typecheck, test on Node 18/20/22)
• Add GitHub Actions publish workflow (auto-publish to npm on release)
• Add Codecov integration with json/lcov coverage reporters

Test plan

• CI workflow runs successfully on push to main
• Codecov receives coverage report
• Publish workflow triggers on GitHub Release creation
