feat(mappings): add sex, gender, and karyotypic ontology mappings

Add curated HRIO mappings for sex, gender, pronouns, and karyotypic
concepts across FHIR, HL7, CDISC, GA4GH, GSSO, SNOMED, and related
sources.

Also add the required namespace prefixes, update the mapping set
version, and regenerate the TTL mappings output.
