various changes using CT-PET-VI-02 #4

* use vector file fig to illustrate datasets  episodes/fig/CT-PET-VI-02.svg,
* tidies up callouts by splitting text and avoid scrolling left and right in the text box,
* updates and adds caption for fig/itk-snap-main-window.svg
