Add Pratik Gandhi attribution to rate limiter import

Summary

Add Pratik Gandhi as co-author on the original cpex-rate-limiter import commit so the copied work relocated from IBM/mcp-context-forge#3809 is properly attributed in this repository.

What Changed

• rewrote the initial rate limiter import commit to include Co-authored-by: Pratik Gandhi <24753546+gandhipratik203@users.noreply.github.com>
• replayed the subsequent rate-limiter commits unchanged on top
• preserved the repository content exactly as it exists on main

Impact

• no code or file-content changes relative to main
• this PR exists only to correct commit attribution/history for the imported work

Validation

• git rev-parse main^{tree} matched git rev-parse HEAD^{tree}
• git diff --stat main HEAD returned no file-content differences
