Docs: README refresh for tier enforcement + bumped ADR count
Adds a 'Tier enforcement' row to the feature matrix covering the gates, the premium GPT-5.5 routing, atomic quota + refund-on-failure, and the retention sweeper. Bumps the ADR count from 19 to 23 with the four new ADRs (020 tier shim, 021 atomic quota, 022 tier-aware model selection, 023 Lemon Squeezy MoR) called out by number.
