docs(changelog): link plugin names to their documentation pages

Each entry whose leading plugin name resolves to a live check- or
notification-plugin doc page now links to it on linuxfabrik.github.io.
Renamed or removed plugins, the `mysql-*` glob and non-plugin prefixes
(tools, basket files) stay plaintext to avoid dead links.
