The Consumer lane asked for a structured "real-project integration finding" form (Discussion #116, the laminr pilot). The other issue forms (bug / enhancement / config / PR template) already exist; this fills the one gap. The form captures exactly the fields that surfaced real bugs in the first pilot: the mount point (caught the non-root-mount break #113), surrounding URL config / middleware (caught the /accounts/login/ DRF collision #114), and whether a custom AdminSite / get_app_list is in play (drove the grouping work #138). Routes security problems to the advisory link, questions to Discussions (via the existing config.yml). Tier 1 — .github/ISSUE_TEMPLATE/ only, no workflow change. Uncontested (.github/ isn't touched by the active frontend branches). Self-merging under the repo-owner's full-tier authorization. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>
