Add "unofficial" option to official field

Describe the problem

We're seeing an increase of feed submissions from individuals who are explicitly providing either a 1. GTFS feed from a timetable 2. translating a data feed into the GTFS format. It would be useful to explicitly set these as "unofficial" feeds and provide more information about who it is supplying them and how often they make changes.

Proposed solution

• Include "false" option to is_official field
• Add "unofficial_producer" string field
• "unofficial_description" string field

We can reuse the note field as unofficial_description. In addition, we can make this field mandatory while adding the feed in the UI. Thoughts?

• What note field are you referring to?
• If you look at feat: Add a Feed Form new conditional fields #1283 right now the description is optional, but we could require it if the feed is unofficial. Thoughts?

We have a note field in the DB. This is populated from the catalog, but it's always empty.

In the API, we have it on the feed table that is common to all feed types.

Good to know! The scope of this original issue was just adding the form fields to see what types of input we get from contributors, I'll add a separate one for actually populating the DB and API with what we see after we've gotten a few responses
