docs: repoint MCP doc links + homepage to createos.sh (DR migration)

Part of the nodeops.network -> createos.sh authority migration. This repo
(github.com/NodeOps-app/createos-mcp, DR 97) feeds the LobeHub MCP listing,
so repointing it moves both links.

- README doc links (Integration docs, Full Documentation):
