chore(code): rename claude -> claude code, format glm name

1. "claude" in the model switcher might not be super clear that it's claude code harness
2. glm model name is wonky

   The formatGatewayModelName suite is the only describe block in this file that still uses ad-hoc it() tests. The team prefers parameterised tests (it.each), and all three neighbouring describe blocks already use that style. Adding one more ad-hoc case keeps the inconsistency alive; converting the block here would close it.
