Fixes to fighting styles & attack mods

* Tidy corrupted fighting styles tables.
* Fixed issues with the temporary mods functions, especially around temp mods to HP.
* Added ^^targetid^^ attribute to those that can be used in attack macro definitions.
* Fixed some rare attack macro bugs.
