Shigellosis - Enhance symptoms data form

Problem Description

After the Disease enum has been modified in (#13927) as part of epic (#13926), the case data form needs to be updated with Shigellosis -specific details

SORMAS will support Shigellosis disease case surveillance

Proposed Solution

• The Clinical signs and symptoms section should be hidden.
• Display the following symptoms under clinical presentation section
  • Asymptomatic
  • Diarrhoea - GASTRO-INTESTINAL
  • Bloody diarrhoea - GASTRO-INTESTINAL
  • Fever - GENERAL
  • Abdominal pain - GASTRO-INTESTINAL
  • Tenesmus - GASTRO-INTESTINAL
  • Sepsis / bacteraemia - GENERAL
  • Seizures - NEUROLOGICAL
  • Haemolytic uremic syndrome - GENERAL
• The existing fields comments, first and its date symptoms should display.
• Add the following symptoms under Complications.
  • Dehydration
  • Sepsis/bacteraemia
  • Seizures
  • Haemolytic uremic syndrome (HUS)
• Onset and offset dates should be visible.
• Comments, first symptom will be visible.
