Salmonellosis - Enhance sample form

Problem Description

After the Disease enum has been modified in (#13915) as part of epic (#13914), the sample and pathogen test form needs to be updated with Salmonellosis-specific details.

Proposed Solution

Acceptance criteria

• Update the sample material list with the Salmonellosis-specific specimens, each carrying its SNOMED-CT reference code.
• The SNOMED-CT code is included on the sample export.
• Restrict the pathogen test type list for Salmonellosis to: antigen detection, rapid test, culture, isolation, IgM serum antibody, IgG serum antibody, multilocus sequence typing (MLST), cgMLST, SNP typing, serotyping, other.
• For the four typing tests, a free-text "test type text" field is revealed:
  • Multilocus sequence typing (MLST)
  • cgMLST
  • SNP typing
  • Serotyping
• The selected test-type text is shown in the sample's pathogen-test list entry.
• A Yes/No option captures "performed by reference laboratory".
