Salmonellosis - Enhance EPI form

Problem Description

After the Disease enum has been modified in (#13915) as part of epic (#13914), the epidemiological data / exposure form needs Salmonellosis-specific behaviour.

Proposed Solution

Acceptance criteria

Exposure form

• The "Shopping for food" sub-setting is shown for Salmonellosis.
• "Shopping for food" details are shown only when the "Shopping for food" sub-setting is selected, and only for Salmonellosis (other Food-borne exposures do not see it).
• Eating-out venues are removed from the Salmonellosis exposure form.
• For travel abroad, prophylaxis adherence and reason for travel follow the "Travelled abroad" sub-setting selection.

Additional Information

The original plan's (from survey template) food consumption history sub-form and the imported case / probable country of infection fields were removed from this feature and are not part of the EPI deliverable.
