(qa-int)Quick-Immunization is asking for Region and District, but can't select it

Bug Description

After enabling the quick-immunization in QA-INT, try to create the immunization for any case. It's asking for region and district, which we can't select.

Reproduction Steps

1. Enable the quick immunization feature in qa-int.
2. Open any case and try to create the quick immunization by giving the doses.
3. A yellow pop-up will be displayed to select region and district
   ...

Expected Behavior

Region and districts should be visible to select.

@raulbob please take a look. Regions and districts are configured in qa-int. is this relying on any other configurations that are lux specific?
