Remove IN_REMOVAL_TEST_DISTRIBUTION.md

Delete the obsolete IN_REMOVAL_TEST_DISTRIBUTION.md planning document. Also relocate docs/exceptions.md to docs/website-src/docs/exceptions.md (100% rename with no content changes) to reorganize documentation under website-src.
