improve: Fix repo URLs, update versions, consolidate workflows

- Fix repository URLs from /empathy to /empathy-framework in pyproject.toml,
  README.md, and docs/get-the-book.md
- Update stale version references in .claude/CLAUDE.md (v1.9.4/v2.2.5 → v2.2.7)
- Remove duplicate PyPI publishing from release.yml (now handled by
  publish-pypi.yml via OIDC trusted publishing)
- Add code-quality job to tests.yml with tech debt threshold monitoring
- Add site/ (MkDocs build output) to .gitignore
