chore: remove orphaned files and cruft

Remove 44MB+ of unnecessary files:
- generate-report.test.mts.backup (32KB test backup)
- DIFFERENTIAL_RENDERING.md (orphaned docs with no implementation)
- .DS_Store files (macOS junk)

These files provided no value and cluttered the repository.
