Integrate new account creation with normal interview flow, instead of directing to a separate interview

Right now, the existing interviews expect you to click a link to make a new account.

This should be integrated into the normal interview flow for most users. It's really just copying/pasting the existing questions into this one.

I'm trying to figure out, without success yet, if we can check if a eFileMA account already exists. It doesn't seem like there's an API for that. If we could it might make the login process a little smoother

Closing the loop: there isn't a built-in way to check if the account already exists, but I'm OK with how this works in MAPetitionToSealEviction
