test(doctor): add doctor module coverage

Ports doctor module coverage onto current main, resolves CodeRabbit review feedback, and updates ide-sync expectations for the skills-first contract. Validated with targeted doctor tests, full local Jest, lint, typecheck, GitHub Actions, Vercel, and CodeRabbit.
