Adjust selection options in the sidebar

Suggestions via @swyatt-SU for the filtering options in the left-hand sidebar (format tweaked slightly):

Replace current selection options with the following six:

1. Explore data for a local authority of hospital?
   • Radio button with two options
2. Select local authority / Select provider trust?
   • Dropdown list, text and drop-down option should change dependant on (1)
3. Hospital setting
   • Three check boxes: inpatient, outpatient, emergency department
4. Mechanism
   • Four check-boxes (prevention, de-adoption, redirection/substitution, efficiency)
5. Type of potentially mitigatable activity
   • Drop down list filtered depending on (3) and (4)
6. Sub-type
   • Drop-down list where a TPMA has several sub-types
     • Options to be updated based on (5)
     • Unavailable if (5) not selected or if the TPMA selected has no sub-type

Note the need to update the names, mechanisms, etc, in the centralised repo, which this repo should feed from:

• Update the mechanisms lookup TPMAs#8
• Update names and types/sub-types in TPMA lookup TPMAs#10

Will need to experiment a bit, considering things like:

• the impact of filter complexity on app bookmarking functionality
• impact of later dataset expansion to ICB (and more?)
• the sidebar becoming scrollable, hiding content
• potentially difficulty in selecting TPMAs from the final dropdown (if selections 'fall off' the bottom of the sidebar, etc)

Awaiting comments/confirmation of the content before completing this task (due 1 July).

Further thoughts:

1. Retain care-shift checkbox?
2. Dropdowns better for space. Radios better at telegraphing options? Checkboxes not quite the same as multi-select dropdown in terms of underlying code.
3. Wording: inputs data is at the level of 'providers'/'datasets'/'trusts' rather than 'hospitals'.

@matt-dray I'm not sure of the provenance or utility of the 'care shift' categorisation. If we keep it then someone needs to make the case for it, and win the debate.

Some updates:

• made headway in branch 200-sidebar
• addressed Use new TPMA lookup #211 by deleting old lookups, reading from the updated one a draft PR in the TPMAs repo: Update TPMA lookup TPMAs#14, and rewiring app variables and flow as a result
• removed old care-shift checkbox and activity-type and category dropdowns, which removed some complicated interdependent filtering
• added checkboxes for activity type and mechanism
• switched to radios for the geography selection
• added subheaders (e.g. 'Inpatients: Hospital Efficiency') into the TPMA dropdown to make navigation a little easier
• experimented with bidirectional filtering (user selects a TPMA and the checkboxes autoselect, or user selects checkboxes)
• experimented with routing with {shiny.router} but I think vanilla Shiny bookmarks may work better now with checkboxes and the removal of the complicated interdependent filter, meaning we don't have to add extra bookmark state handling on app startup

Next:

• disable TPMA selection if selection permutations aren't viable
• integrate the bidirectional filtering and setup bookmarking so we only need to record the unit and TPMA (which will backfill the checkboxes)
• break out the sub-types into a separate filter
