feat Implement structured output revamp with PydanticAI-style interface

Refactor structured output to integrate with main agent loop, allowing
output types to be specified directly in agent calls. Remove separate
structured_output() method in favor of unified approach that supports
proper metrics collection, tool usage, and event streaming. Clean up
planning documents and outdated examples after implementation.
