Closes agentevals-dev#96 score_model scores agent responses using a model judge, no golden set needed. Config requires model and input. range defaults to [0, 1].

Adds score_model grader type to the OpenAI Evals backend.
score_model scores responses without a golden set.
The model reads each response and returns a float. Config needs model and input. range defaults to [0, 1].
