kubedb: make it a package of the client module, not a nested module

Per review: the kubedb provisioning client should be part of the
go.bytebuilders.dev/client module (used by the bmchelix and servicenow
repos), not a separate nested module. Remove kubedb/go.mod and add the
Apache license header to each file so it lives as go.bytebuilders.dev/client/kubedb.
