support mixed in-order recording and replay (#132)

* remove guards for cl_khr_command_buffer_mutable_dispatch

We require newer headers anyhow, so we may as well remove these
ifdefs to clean up the code a bit.

* add support for mixing in-order recording with out-of-order replay

* bump command buffer version to 0.9.7
