fix(database): filter recent-activity on updated_at instead of created_at

Notes edited after their creation were excluded from recent-activity results
because both SQLite and Postgres search repositories filtered `after_date`
against `created_at`. Changed both to filter on `updated_at` so that
recently-modified notes are always included regardless of when they were
first created.

Also fixed Postgres unranked ORDER BY to place `updated_at DESC` before
`id ASC`, ensuring newest-updated results come first for timeframe-only
queries (no text search).

Adds a regression test that directly indexes an entity with old `created_at`
but recent `updated_at` and verifies it appears in results, and that a stale
entity (old `updated_at`) does not.
