slack.gliderlabs.com is dead (hangs and times out), so both the badge and the "join our Slack" line at the bottom of the README went nowhere. Pointing them at the Charm Discord at charm.land/chat, which is what the rest of the charmbracelet repos use. Closes charmbracelet#40. Signed-off-by: Charlie Tonneslan <cst0520@gmail.com>

`slack.gliderlabs.com` is dead (the host just times out), so both the badge at the top of the README and the "join our Slack" line at the bottom went nowhere. Pointed them at `charm.land/chat` (the Charm Discord) since that's where the rest of the charmbracelet repos point.
One-line README change, no code touched. The rest of the README still has Gliderlabs branding from before this repo was rehomed — happy to do a follow-up PR cleaning that up too if you'd like, but kept this one tightly scoped to the dead link.
