fix: update bug report template for openagent

This pull request updates the bug report issue template to reflect the project's new name, changing all references from "oh-my-opencode" to "oh-my-openagent". It also updates related links, instructions, and descriptions to ensure consistency and accuracy for users submitting bug reports.

Project naming and documentation updates:

• Updated all references in .github/ISSUE_TEMPLATE/bug_report.yml from "oh-my-opencode" to "oh-my-openagent", including the description, checkboxes, and placeholders.
• Updated documentation and policy links to point to the correct "oh-my-openagent" URLs.
• Changed instructions and placeholders for steps to reproduce and doctor output to use "oh-my-openagent". [1] [2]
• Updated example outputs and configuration descriptions to reference "oh-my-openagent" instead of "oh-my-opencode". [1] [2]

Template usability improvements:

• Made the "latest version" checkbox optional instead of required, allowing users to submit bug reports even if they're not on the latest version.
