fix: repair 11 broken cross-references + add constructive-pnpm to indexes

- Fix 3 broken refs to ./references/search.md in constructive-sdk-graphql/SKILL.md
  (file doesn't exist; redirected to search-composite.md which covers the same content)
- Fix 8 broken ../constructive-* paths in constructive-platform/references/
  (files inside references/ need ../../ to reach sibling skills, not ../)
  Affected: blueprints.md (2), storage-policies.md (4), blueprint-definition-format.md (2)
- Add missing constructive-pnpm to AGENTS.md and CLAUDE.md skill tables
  (17 skill directories but only 16 were listed)

All 284 cross-references now validate successfully.
