Investigate missing value set

Describe the bug
Missing ValueSet http://cts.nlm.nih.gov/fhir/ValueSet/2.16.840.1.113762.1.4.1190.38 when attempting to run the NHSN measure
Definitely missing from the bundle, even though it is present in the dependencies and data requirements lists for the measure
Added to the vocabulary folder to get past testing, but need to investigate why this isn’t in the bundle

The issue is that the measure was not present when we prepared the manifest, so the value set was not included in the expansion package. Rebuilding the manifest with the measure included as a component will address the issue.
