Algorithm violations in PAI 5.0, anybody?

In less than a few work sessions, my DA violated the Algorithm three times -- twice by skipping step 4, once by jumping head first right to 7. I suggested it investigate and figure out what was going on and fix it, and it's coming up with a 6.4.0 🫣 version. I'll get a write-up of what was done and share (if it works 😅) but I wanted to know if this kind of "playing loose" with the Algorithm was something others had noticed, or had to deal with, or tried to fix.
