First printing, plus a file to generate malignancy info

This release adds a file not directly referenced by the book that generates the malignancy information that we use in chapter 14.

First printing

This is the state of the code as of when the book went to print in June, 2020.

The main branch of the code will receive errata and updates, and so might drift from the printed page.
