docs: restructure README with One-Command Install first

- Move One-Command Installation to top of README (was buried at line 212)
- Reduce README from 732 to 299 lines (-59%)
- Add Table of Contents with links to docs
- Create docs/CONFIGURATION.md - models, env vars, system requirements
- Create docs/TROUBLESHOOTING.md - common issues and solutions
- Create docs/IDE-SETUP.md - manual IDE configuration
- Add links to language analyzer READMEs (Go, PHP, Laravel, Python)
- Add link to workspace detection docs
- Preserve all SEO keywords and MCP discovery elements
- Keep llms.txt reference for AI agents
