Standardized Modal Structure

Modals are used in a variety of areas within the UI. The format of these modals needs to be standardized to ensure consistency. As bootstrap is the standard CSS framework in the UI, it makes sense to start with the bootstrap-modal as the base.

• have a complete mock of a modal including header, body content, footer, button positioning

Note for me. We need to get ghlabels attached to this repo so that we can use our standard flags. Additionally, this is a blocker and we'll need some design effort such that @andrew-c-tran can then back this into the style guide and ultimately the ddev-ui.
