Extension features not working/crashing with errors in ExtensionService

Hi, for about a couple of weeks I've been having some issues configuring and using this extension. Anything "advanced" like going to declaration/definition etc. Only thing that is working looks to be syntax highlighting. There isn't a an error in the ElixirLS output. However, in the ExtensionService output, there is a bunch of errors like this:

I tried completely reinstalling (including clearing any residue) of: VSCode, asdf, Elixir, Erlang, recompiling the project...

I can't pinpoint the version when this started, or rather stopped working, but 0.28.0 isn't logging any errors and by the looks of it is working better (although a bit flaky).

Cannot reproduce. Extension is working as expected on my machine and the errors you are seeing are harmless

Upon further investigation this may be an issue. Are you running a multiroot workspace?

No, not really. Everything is in one directory but with an umbrella project and subprojects. It's private but if it would help I can share the structure with you or obfuscate before uploading, since it is kind of a big project and there might be something else breaking it.

This is happening to me at the moment and while I'm not "intentionally" running a multi-root workspace, I can see in the Developer Console, Workspace is undefined but Workspace folders are a single element array of type WorkspaceFolder. Inside the extension's activate() function, the WorkspaceTracker mode has been set to SINGLE_FOLDER but I cannot tell wether or not that is correct in this case. I am also running an umbrella project that contains a "moderate" number of apps. The root of the repo contains a mix.exs file.

The errors mentioned above repeat several times and while this is not a problem during extension installation, it seems to be multiple "instances" of the extension running in parallel during general use and multiplying the warnings and errors and problems that are detected, and generally slowing down VSCode to a crawl. One thing that I noticed is the following log repeats many times in between other "real" log statements:

ElixirLS: Registered language model tools for client

And the errors appear in a complete "set" under each instance of that repeating log statement. Maybe that is a clue?

Running unit tests seems to hang, as if the extension is continuously spawning although I have not confirmed this, but based on the logs in the Developer Console it looks like the test process eventually became unresponsive. I ran out of time to spend investigating but I am hoping what I have described provides some helpful information. For now I need to downgrade from 0.30.0 to 0.29.3 as a workaround. Let me know if I can provide more information to assist.

Please share a minimal repro. There were no significant changes on the VSCode lately
