…icate with index Mirror upstream's diagnostic: 4 messageIds (missingTitle, emptyTitle, dynamicFalseTitle, duplicateTitle). Every iframe with a duplicate title is reported with cross-index, matching upstream's behavior for 3+ duplicates (port previously reported the first occurrence only once).
