Improve error-handlers

High level improvements:

• Make sure the impl follows the error-handler pattern established.
• Better improve the output from the log-and-continue default error handler
• Improve assertions+validation of fn input (eg, the re-frame.core/reg-sub signal destructuring) - make it something that can be disabled for a production build
