fix: update vulnerable step-security/harden-runner and stale action refs

Update step-security/harden-runner from v2.15.1 to v2.16.0 to resolve
known-vulnerable-actions findings, and update kenyonj/mark-ready-when-ready
to v1.1.3 (tagged release) to resolve stale-action-refs findings.
